Chandra Smith

Chandra Smith, MSN, APRN, FNP-C, is a board-certified Family Nurse Practitioner with dual licensure in Illinois and Wisconsin, credentialed by the American Association of Nurse Practitioners. She earned her Bachelor of Science in Nursing from Rockford University and completed her Master’s of Science in Nursing at Olivet Nazarene University. With a diverse and extensive nursing background spanning across multiple care settings, Chandra has honed expertise in integrative, regenerative, and functional medicine, as well as hospice and palliative care, acute hospital and long-term care, school nursing, and nursing education. Additionally, she is certified in bioidentical hormone pellet therapy.
After dedicating much of her career to palliative and hospice care, Chandra developed a deep passion for preventative medicine. Her own personal health journey led her to explore alternative medicine, where she found healing through functional and holistic approaches. This transformative experience sparked her passion for helping others find the root causes of their health concerns. She has since pursued further training in functional medicine, with a particular focus on alternative cancer treatment, metabolic disorders, and infertility.
Chandra is committed to empowering her patients with the knowledge and tools to make informed decisions about their health. By focusing on root cause medicine and lifestyle education, she provides compassionate, personalized care that addresses each individual’s unique needs. Her holistic approach bridges traditional, preventative, and functional medicine to meet patients where they are in their health journey.

Nicole serves as the Business Manager at our practice, overseeing daily operations, improving workflows, and ensuring every aspect of the patient experience runs smoothly. By developing efficient systems and streamlining behind-the-scenes processes, she helps create more time for our providers to focus on what matters most—our patients.
In addition to managing practice operations, Nicole assists with patient triage, helping answer clinical questions, coordinate care, and support patients throughout their health journey. Her passion for organization, patient advocacy, and continuous improvement helps ensure each patient receives the personalized attention our practice is known for.

Melissa
Melissa is often the first friendly voice patients hear when contacting our office. As our Patient Care Coordinator and Virtual Receptionist, she assists with scheduling appointments, answering incoming calls, coordinating communication, and helping patients navigate their care with ease.
Melissa is dedicated to creating a welcoming experience from the very first phone call. Whether she's helping a new patient schedule their first consultation or assisting an established patient with their next appointment, she strives to make every interaction efficient, compassionate, and stress-free.
